Critical Illness (Lump Sum) PTSD Benefit Full-time/Part-time Employees and Volunteers
Maximum Limit each diagnosis and Lifetime Maximum Benefit: $3,000
Optional Benefits Available for Additional Contribution:
$5,000; $10,000; or $15,000
Long-Term Disability (Income Replacement)
Elimination Period: Benefits begin 90 days after becoming disabled due to occupational PTSD
Benefit Duration: 36 months provided the first responder is deemed disabled due to occupational PTSD
Benefits (employed first responder):
Full-time/Part-time Employee: 60% of pre-disability earnings as an eligible first responder for Participating Employer
Minimum Monthly Benefit: $100
Maximum Monthly Benefit: $5,000
Benefits (volunteer first responder):
Long-Term Disability Income $1,500 Monthly Benefit
Optional Benefit Available for Additional Contribution:
$2,000 Monthly Benefit
